Which GLP-1 produces the most weight loss?

Across published trials, Retatrutide shows the highest average weight loss at 24.2% (Phase 2 (NEJM 2023)), followed by Tirzepatide at 20.9% and CagriSema at 20.4%. Only Tirzepatide and Semaglutide are FDA-approved — every other compound in this table is still in trials, so its figures come from study populations rather than general use. Trials differ in length, dose and participants, so these percentages rank the compounds but are not strictly like-for-like.

These percentages come from separate trials that are not directly comparable: they ran for different durations, enrolled different populations, and used different titration schedules, so a two-point gap between two compounds may reflect trial design rather than the drug. Most of the table is still investigational — figures from a supervised trial are a poor guide to unsupervised use.
